Staff of the Plague Lady

 
The Plague Lady, as she's known, and the events that surround her are dark and twisted. She got her name after an episode where her husband was killed, hundreds of years ago, in the forest where she still resides. She got so angry and sad that she turned the forest into a swamp, she turned into a Hag and the staff became chaotic with necrotic magic. In her anger she killed her husbands attackers with wicked diseases, and their remains still lie around the forest where she lives.
  Once per turn you can expend a charge to force a creature, within 30 ft, to make a DC 15 Constitution saving throw or be inflicted by a random Disease. Roll a 1d6 to see which disease you inflict. The symptoms of the disease take effect on the creatures next turn.   The magically created diseases themselves, and their symptoms, last for 24 hours. The won't spread to nearby creatures.   It is possible to affect the same creature with more than one disease. Also the numeric values (damage, exhaustion level, penalties) of the diseases can stack if you affect the same creature with the same disease more than once.

(1-2) Cackling Fever

  • On a failed save, the creature takes 5 (1d10) psychic damage and becomes Incapacitated with mad laughter for 1 minute. The creature can repeat the saving throw at the end of each of its turns, ending the mad laughter and the incapacitated condition on a success. After a success the symptoms won't take affect for another 2 hours.
 

(3-4) Sewer Plague

  • The infected creature suffers 1 level of Exhaustion. The creature can repeat the saving throw at the end of each of its turns, removing 1 level of exhaustion on a success. After a success the symptoms won't take affect for another 2 hour.
 

(5-6) Sight Rot

  • The creature takes a −1 penalty to Attack rolls and Ability Checks that rely on Sight. At the end of each turn, the penalty worsens by 1. When it reaches −5, the victim is Blinded. The creature can repeat the saving throw at the end of each of its turns, removing 1 level of sight penalty on a success. After a success the symptoms won't worsen for another 2 hours.
