Regeneration. The troll regains 10 hit points at the start of its turn. If the troll takes acid damage, it regains only 5 hit points at the start of its next turn. The troll dies only if it is hit by an attack that deals 10 or more acid damage while the troll has 0 hit points.
Threatening Reach. The troll can make opportunity attacks against all enemies within its reach (10 feet).
Many Limbed. The trolls extra limbs makes them capable of several opportunity attacks per round. 2 for 4 armed. 3 for 6 armed.

Actions

Multiattack. The troll makes four or six Claw attacks.   Bite. Melee Weapon Attack: 1d20+8 to hit, reach 5 ft., one target. Hit: 14 ( 2d6+6 ) piercing damage plus 9 1d8+4 acid damage   Claw. Lair Actions

Shifting Sands. 10ft by 10ft. area of sand churns and swallows up anyone standing in it. DC 13 Dexterity save to avoid being incapacitated.
Escape: Action to dig your feet out of the sand again.

Sandy cleft in a desert. Hiding and living quarters in the surrounding rocks.

Sand trolls from the deserts of Karnor.
Many armed, ill tempered beasts, that usually live in groups of 3 to 7 individuals.
They experiment with using their regenstion to augment their bodies by severing limps and attaching them again in new places.
This means they will usually have 4 or 6 arms and the older members of the clans are usually also covered in spikes made from their own bones.
